For all physician appointments, please bring a list of all medications, including the dose and schedule, or bring your medicine bottles. The best time to refill prescriptions is at your appointment.

Please call our prescription line for medication refills before your medication runs out. Note: This line is managed during regular business hours, from 8:00 a.m. to 4:00 p.m.

Make sure you have the following information at hand when you call to refill a prescription:

  1. Patient’s first and last name. (Please spell out name.)
  2. Date of birth
  3. Telephone number where you can be reached, if necessary
  4. Name of the medication (be ready to spell it), the dosage, and directions
  5. Will the prescription be mailed, picked up, or phoned into your pharmacy?
  6. If phoned into your pharmacy: Pharmacy name and location
  7. If prescription is to be picked up, let us know: A) Name of person picking up the prescription (IMPORTANT: Photo ID is required for pick up); and B) At which location the prescription will be picked up
  8. If the prescription is to be mailed, leave a complete mailing address.

Notes:
• Some prescriptions require that they be hand-written, not phoned in.
• If you don’t hear back from the nursing staff within 24 hours, your prescription has   been filled.
• If you request to have the prescription mailed, it should arrive within 2 to 3 business days.
