Please arrive at our office approximately 15 minutes prior to your scheduled appointment to complete any additional laboratory tests and unfinished paperwork. Please bring all of your insurance cards and any required referrals with you. Since our business office will file your insurance claims, we ask that you present your insurance card each time you visit our clinic.

ESTABLISHED patients who need X-rays prior to their visit with the doctor, should register with the hospital where the films are to be done 15 minutes before the X-ray. After the X-ray you should arrange to arrive at our office 15 minutes prior to your visit for laboratory tests. If no X-rays are required you should arrive to our office no earlier than 15 minutes prior to your visit for laboratory tests.
