SUMMARY OF DUTIES: Responsible for maintaining all medical records for patients of the clinic. While working with the Electronic Health Records (EHRs) system, this person creates and retrieves electronic patient folders/records, reconciles and resolves medical record discrepancies, accesses electronic files, updates electronic records/documents, prepares medical records for scanning and indexing. Responsible for making sure all medical records are up to date and verify that all information is accurate as well as manage the release of medical records in accordance with HIPAA regulations.

- Monitor and manage incoming department/patient documentation, including In basket Messaging, Quality Checklist (QCL)/Worklist, patient emails and electronic faxes.
- Prepare charts for office visits
- Retrieve patient medical records for physicians, technicians, or other medical personnel via fax, mail, and or other ancillary EHR systems throughout the country.
- Scan manually and or electronic records to/from EHR system
- Protect the security of medical records to ensure that confidentiality is maintained.
- Review records for completeness, accuracy, and compliance with regulations.
- Release information to persons or agencies according to regulations.

QUALIFICATIONS AND EXPERIENCE:
- High School Diploma or GED
- Experience in records management preferred
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of the Health Information Portability and Accountability Act (HIPAA) in regards to release of medical records.
- Knowledge of clinical policies and procedures.
- Knowledge and experience in customer service concepts and techniques.
- Knowledge of baseline computer functions and ability to operate office equipment including phone, computer, fax machine, scanner and photocopying machine.
- Ability to establish/maintain cooperative relationships with patients, their families, physicians, staff and others.
- Ability to problem-solve and use judgment when performing work responsibilities.
- Ability to read, understand and follow oral and written instructions.
- Ability to respond to stressful/emergency situations in a calm and effective manner.
- Ability to multitask and manage several complex issues at one time; prioritization.
- Willingness/ability to adapt to the circumstances in the environment.
- Ability to communicate efficiently with individuals and groups, in writing, or by phone.
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT
- Occasionally lift up 10 lbs. Rarely push/pull up to 25 pounds.
- Regularly required to stand; sit; climb, balance, stoop, kneel, crouch or crawl; twist; bend; walk; use hand to finger, handle, or feel; reach with hands, arms
- Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and to adjust focus.
- Sit for prolonged periods of time.
- Work is generally performed in an office setting; desk/cubicle with chair, computer and phone
- The noise level in the work environment is usually quiet to moderate
